Yes the player has the choice not to enter his/her main phase/battle phase but the situation he is saying here, his opponent will want to. The Draw, Standby, and End must be entered.

Phases are entered no matter what the field is, its just that you may not be able to do anything in those phases because of what is or isn't on the field.

All answers are confirmed. And you are correct about why Nobleman of Extermination fizzles. As Nobleman of Extermination says the trap/spell must be facedown for it to be activated, chaining to an activation is something that "happens before" the Nobleman of Extermination is activated. So...

So then, I'd assume, the effect of Sinister Serpent could still be activated.

I also thought this was the ruling for the same reason why when you destroy a Mystic Tomato/Shining Angel/Giant Rat/Mother Grizzly/etc that you cannot activate Torrential Tribute on the monster that they Special Summon.
