You can only activate this card when a monster with an ATK of 1000 or less is Normal Summoned, Flip Summoned or Special Summoned. Remove the monster from play and your opponent removes all monsters of the same name in his/her hand and Deck from play. His/her Deck is then shuffled.
Rulings
You can activate "Chain Disappearance" in response to your own Summon of a monster, in which case your opponent still removes all cards with that name from his hand and Deck.
If you use "Chain Disappearance" in response to a Flip Summon, any Flip Effects like "Penguin Soldier" would still resolve. The Flip Effect is Step 1 of the chain, and "Chain Disappearance" is step 2.
The effect of "Chain Disappearance" can remove multiple monsters that are Summoned simultaneously, for example if your opponent activates "Scapegoat", you can respond with "Chain Disappearance" to remove all 4 "Sheep Tokens" that were Special Summoned from the field.
If your opponent activates "Chain Disappearance" and you chain "Ring of Destruction", "Ring of Destruction" resolves first and destroys the monster and sends it to the Graveyard, and there is no monster of the same name so no cards are removed from play for the effect of "Chain Disappearance".
If you use "Chain Disappearance" when a Fusion Monster is Special Summoned, remove from play that Fusion Monster plus all copies of that Fusion Monster in the Fusion Deck.

Chain Disappearance a card underrated by a lot when it was released but now it's getting a lot more attention.
I got this card twice in 2 IOC pack and i was like "what a waste of card" but when i started analyzing it more I could see how great the card was.Chain disapearance may need a monster with an atk of 1000 or less to be summoned but it destroys it plus destroys all other in deck and hand but wait it doesn't destroy the monsters it removes them from play and most decks wont be able to get the monster back from the remove from game area. People has realized what a great card chain disapearance is especially in today's advanced meta with Magician of Faith and thousand eyes restrict being used by most people.In addition if your opponent says he doesn't have that monster in his deck or hand you get to check his hand and deck in order to proof it(thanks to densetsu_x for confirming).And if you check the rulings you will see why this card is very useful againts chaos decks.......
