Anything you draw is considered to be "in your hand" and that is important for some effects (i.e. Night Assailant, ALO, Cost Down, etc.) Cards that are just "picked up", like from Cyber Jar's effect or "Selected" like Painful Choice, are never considered to have made it to the players hand, so certain effects will not activate.
 
When I a card says draw from your deck don't pick up. Does that mean that you can pick up one card at a time our do you have to pick up more then one card at a time?
 
Generally, an effect that isnt putting cards directly in your hand at activation, will tell you to "pick up" cards and discard at resolution.

Examples:
Painful Choice
Morphing Jar #2
Monster Gate
Cyber Jar


Effects that activate and tell you to "draw" cards, usually leave you with a card(s) in hand.

Examples:
Pot of Greed
Mirage of Nightmare
Morphing Jar
Graceful Charity


Basically, if you are told to "pick up cards", these cards are not considered in hand unless the effect specifically instructs you, or your opponent to select or choose a card to remain in hand and most likely all others will be sent to the Graveyard and are only eligible for effects that send cards "from the deck to the Graveyard".

If you are told to "draw card(s)", that card or cards will remain in hand and if an effect tells you to discard or send a card, it will have been sent from your hand and is eligible for effects that send cards "from hand to the Graveyard".

One card at a time is the normal way to prevent picking up more cards than instructed to do so, since that is a major penalty.
 
The reason I asked I have built a deck that pulls cards out of the graveyard if it said pick up I could pick up like five cards and the fifth card would be the card effect and the other four would go to the graveyard.
