Archfiend of gilfer's effect activates in the grave as per it's text... so why doesn't it come back every time it is sent to the grave, including being sent to the grave while considered a magic card...
 
You just answered the question in your own question. Because its a Spell Card when its sent if it was previously attached to a monster.

It's different from Necrofears effect which states that it is not an optional effect if it is destroyed.

Gilfer's effect is optional, which means that it is triggered and you must be able to perform the action immediately, so going to the Graveyard has to be the last thing that occured or its effect disappears.

Necrofear's effect kicks in after all other effects have resolved first.
 
Yes it's a timing issue, but it will come back to the field even if it is an equip card at the time and sent to the graveyard (like during the battle phase... you attack and destroy a monster that Gilfer is equipped to, you can bring him back and equip him to another of your opponent's cards, attack that one, bring him back to equip to another, etc. as long as your opponent has enough face up monsters).
 
As mentioned it can come back if it's the last action to happen in the turn. Monster, spell, whatever. Because of that, it is optional.

For instance, it can't come back if by doing so you or your opponent would gani/lose life points by removing it as an equip.
 
If they were mandatory effects, I could see that. But would you be able to say that you are activating 3 optional effects at once? I'm not so sure.
 
Assuming "Miracle Dig" returning cards to the Graveyard can be considered "Sent"...

Since all 3 return at the same time, the timing is correct to activate all 3 effects which then would go on a chain.

An easier example (though the card is banned): "Painful Choice". If 2 or 3 are sent to Graveyard with Painful Choice, the timing is ocrrect and you can activate each one's effect and bring it back.

Seems my thinking might be wrong here.

The old Netrepâ„¢ Files 6.0b shows 2 rulings that might actually show they are sent through Miracle Dig.

"Banisher of the Light" vs. "Miracle Dig" - "Miracle Dig" may not be activated while "Banisher of the Light" is on the field.
Considering the text of Banisher, and that Miracle Dig cannot be activated, it seems to suggest that they are infact "sent" by Miracle Dig.

"Dimensionhole" vs. "Miracle Dig" - Monsters removed from play by the effect of "Dimensionhole" may be sent to the Graveyard by the effect of "Miracle Dig". The effect of "Dimensionhole" will then Disappear.
It uses the wording of "sent" but there have been many cases of incorrect terminology used in these files and the FAQ.

Of course these are "old" rulings, new info might actually change this.
