You have Question as one of the cards you play in your deck. As far as Im concerned, you cant really tell me that you actually remember all non monster cards that came before your first monster was sent to the Graveyard, and in the exact order.
Is that an example of a damaged game state that would be irrepairable? Do you think that puting your Graveyard back together fixes things, or makes it worse since you know there is no effect that allows for your Graveyard to be manipulated? It benefits you more than your opponent since you can alter the bottom monster card if you honestly dont remember, but maybe he does.
In the case of picking up two cards, nothing has really changed except that both players now know what the card is. First time, a warning. Second time, intentional or not, the penalty must be more severe.
